Your First Visit:           
We ask that you arrive approximately 15 minutes before your scheduled appointment time. During this time you will be asked to fill out the necessary paperwork.  No matter what kind of insurance you have you will be asked to fill out a medical history form, and a consent to treat form. Both of these forms are located below and available for download if you wish to print at home and fill out prior to arriving for your initial evaluation. An additional form is also available for download specifically for patients who have Medicare as their primary insurance. Please don't be confused as the title of the form is "Medicare as a Secondary Provider."  Medicare requires that this form be filled out. Upon arrival you will be asked to provide your driver's license for identification purposes, insurance card(s), and the prescription if you have one. Please dress comfortably and anticipate on the first session lasting about an hour and a half. 

Insurance:  
We participate with most medical insurance carriers, auto and workmans compensation claims. To the right is a list of the most common carriers. Please call the office if you do not see one on the list to check if we participate.

Copayment and Payment Information:  
You have options when it comes to making payments on your account. We accept cash, check, and credit. Credit transactions must be made using a Visa, Mastercard, or Discover card. Payment can be made in office or  over the phone.
